Install older style VS extension

I have a Visual Studio extension (the old format, not the VSIX). Visual Studio doesn't display the extension anymore.  How do I reinstall the extension? (when the extension was created (w/VS2012), a MSI wasn't created (not even an option anymore).

I tried devenv.exe /resetaddin myaddin.Connect, but it still didn't load.
