Issue with my VS extension pacakge

Hi,

        I have created a vs menu extension package, and its works fine when i am using it from source, but when i am installed the vsix , i get the error saying 

          "The package dint not loaded correctly"

The problem may caused due to configuration changes or by installation of other extension,you can get more info by examining the following file

C:\User\Username\Appdata\Roaming\Microsoft\VisualStudio11.0\ActivityLog.xml.

I can't find the root cause.what was wrong with my addin? please provide me a solution asap.
