vsix not installing properly

I have a number of VSIX projects to create project templates. I modified a file in one of my templates and exported the project. Then I built my VSIX project. I went to the extension manager in VS 2012 and uninstalled my current extension. Then I exited VS 2012 and ran the vsix file. It said it would install into VS 2012 Premium and I said OK and it installed.

 I opened VS 2012 and ran file/new project. But my templte (a VB .NET one) does not show up. I go back to the extension manager, and it is listed there. I can unzip the vsix file and the project template zip file it contains with no problem. I even rebooted the machine hoping something was preventing the installation and the did all this again. If I try to install twice without running extension manager in between, the vsix installer tells me it is already installed. So it seems to almost be installed but it just doesn't show up.

 What should I examine to see why this isn't working? I would also note that my previous .vsix file is in TFS. So I fetched it out and tried it again - that one fails too. And I know it was a working file just last week when I last updated my template project and uploaded it to the on-line gallery. So something seems to be wrong other than the vsix file I produced today. Perhaps something in the registry or on disk?
