How do I add .addin options page to Shell Isolated ?

I am reading this article on MSDN. I am able to add an options page by using this method to VS 2012. But how do I add options page this way to my Isolated Shell project ? I have tried placing the .addin files in all mentioned directories.

I know how to add options page through VSPackage. But I need to do it this way.
