ProjectItems.AddFromTemplate() has stopped firing the wizard and now tries to add the template XML to the project as a file.

I am building a custom VSIX package with an ProjectItemTemplate that includes a wizard. At some point today, after a minor tweak and re-build the package started adding the XML (item template) file to the solution instead of running the wizard specified in the XML file. I can't debug it because when I call AddFromTemplate this is what it does. There is no other code to debug.
